Costs

Upgrades come with a few associated costs. The first is the cost of performing the upgrade. This price is contingent on the new ilvl of the item and the amount of perks it gains and can be costly. It is also costly to upgrade an item to a higher level (such as Explorer > Champion or Veteran > Heroic).

The second is the price of the materials needed to upgrade. The items used for upgrades are usually either Power Dices (dropped by monsters 21and up) or Minerals and Erons (obtainable by completing specific Mists of Pandaria or Warlords of Draenor quests). The base stats of an upgraded item are calculated based upon its new ilvl and other properties. You can also add additional materials to the upgrade to increase the chance of success. The materials are displayed in a separate window above the + button. However, there are some limitations to this, and the highest amount of material that can be added is shown right above the + button in the user interface.

Another method to increase the odds of an upgrade succeeding is by using an item called a Blacksmith Blessing or an upgraded version of it.
