Certain items come with inherent boo, which is not effective when upgraded, such as the Goldwrap which provides extra potion slots. These are usually upgraded to add value to a close-to-perfect or perfect item.
Costs
There are some costs associated with item upgrades. The first is the upgrade itself. The cost for this upgrade can be costly and is dependent on the new ilvl of the item and the number perks it gains. It can also be expensive to upgrade an item to a better tier (such as Explorer > Champion, or Veteran > Heroic).

A Blacksmith Blessing or an upgraded version of it, can be used to boost the likelihood of an upgrade succeeding. This item level upgrade is useful if your weapon or piece is nearing or has reached the maximum level of its level. This item lets you upgrade the equipment to the next tier without the risk of it breaking.
If an upgrade fails, it will be reverted to its original state. This is often an upgrade, but it could also be an upgrade. Save an upgrade until you are nearing the maximum level of an item.
