What is an Item Upgrader in Pokemon Go?

The item upgrader is an additional widget at the top of both the Angel and Cash Upgrade Windows. The Manager is priced at 33 billion Angel Investors and offers no gameplay benefit aside from making it easier to find and buy upgrades.

Some items come with built-in boos which is less useful when upgraded. For example, the Goldwrap offers an additional potion slots. These are usually upgraded to add value to a close-to-perfect or perfect item.
Costs
There are some costs associated with item upgrades. The first is the cost of performing the upgrade. This price is contingent on the new ilvl of the item upgrade and the number of perks it gains and is often expensive. It is also costly to upgrade an item to a better tier (such as Explorer > Champion, or Veteran > Heroic).
The second is the cost of the items required to upgrade. These are typically Power Dices (dropped from monsters 21+), Minerals and Erons or specific Mists of Pandaria or Warlords of Draenor Quests. The amount of these materials needed can be found in the inventory of an upgrade vendor.
Upgrading an item is an intricate process that requires time. It can take 8 hours depending on the size of an upgrade. The base stats of an upgraded item are calculated based on its new ilvl and other characteristics. This calculation determines new Armor Class bonuses, damage dice, as well as other vital statistics. In certain cases, the upgrade will grant additional perks that can be selected from a list of available advantages.
Rarely, the new benefits obtained will cost more than one perk slot In these cases the upgrade will replace the most expensive perks. You can also add additional materials to the upgrade to increase its chances of success. These materials are displayed in a window located above the + button. There are a few limitations. The maximum amount of material you can add is displayed directly above the + button.
Another way to increase the chances of an upgrade achieving success is by using the item known as Blacksmith Blessing or an upgraded version of it. This item can be helpful in the event that your item or weapon is nearing or has reached the maximum level of its tier. It will allow you to upgrade that piece of gear to the next level without risking it breaking.
If an upgrade is unsuccessful, it will be reverted to its previous state. This is usually a break, but could also be a downgrade. Keep an upgrade in reserve until you are nearing the maximum level of an item.
