Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes cleaned trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that compare treatment effect estimates across trials with different levels of pragmatism.

Background

Pragmatic studies are increasingly recognized as providing real-world evidence for clinical decision-making. However, the use of the term "pragmatic" is not uniform and its definition and evaluation requires further clarification. The purpose of pragmatic trials is to guide the practice of clinical medicine and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. Furthermore, trials that are pragmatic must focus on outcomes that matter to patients, like the quality of life and functional recovery. Methods

In a practical study, the goal is to inform policy or clinical decisions by showing how an intervention could be integrated into routine treatment in real-world contexts. This is distinct from explanation trials that test hypotheses about the causal-effect relationship in idealized settings. In this way, pragmatic trials could have less internal validity than studies that explain and be more susceptible to biases in their design as well as analysis and conduct. It is important to remember that a pragmatic study should not mean that a trial is of poor quality. In fact, there is an increasing number of clinical trials that employ the term 'pragmatic' either in their abstracts or titles (as defined by MEDLINE but which is neither precise nor sensitive). These terms could indicate that there is a greater appreciation of pragmatism in abstracts and titles, however it isn't clear whether this is reflected in the content.

Conclusions

As the importance of real-world evidence grows commonplace and pragmatic trials have gained momentum in research. They are clinical trials that are randomized that compare real-world care alternatives instead of experimental treatments under development. They have patients that more closely mirror the ones who are treated in routine care, they use comparators which exist in routine practice (e.g. existing drugs), and they depend on participants' self-reports of outcomes. The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described as pragmatic. They assessed pragmatism by using the PRECIS-2 tool that includes the eligibility criteria for domains, recruitment, flexibility in adherence to intervention and follow-up.
