It could be a weapon that has more damage range, an improved speed of reloading, or increased defense roll.

Costs

There are a few costs associated with item upgrades. The first is the upgrade itself. The cost is based on the new ilvl of the item and the number of perks it will gain, and can be expensive. Upgrades to a higher level (such as Explorer to Champion or Veteran to Heroic) can be expensive.

The other is the cost of the materials needed to upgrade. These items are typically Power Dices (dropped from monsters 21+), Minerals and Erons or a specific Mists Of Pandaria or Warlords of Draenor Quests. These materials can be found in the inventory of an upgrade vendor.

Rarely, the benefits gained will cost more than one perk slot; in these situations, the upgrade will only replace the perks with the highest cost. Sometimes, the upgrader will make other adjustments to an item, for example, changing the type of weapon damage or removing or adding an unwieldy or operational property. These modifications are based on the item's ilvl and may require special materials, such as Adamantine.
