Costs
Upgrades have certain costs associated with them. The first cost is the upgrade itself. The cost for this upgrade can be costly and is contingent on the new level of the item as well as the number of perks it receives. The other is the cost of materials required to upgrade. Success Rate

An Upgrade Pity is a product you can purchase from the smithing NPC in Lost Ark. It increases the chances of an upgrade being successful by a certain percentage. It is similar to the "luck bonus" that certain items provide however it is more reliable. This can be added to any weapon or armor and can be used three times.
You can also add additional materials to the upgrade to increase the chance of success. These are displayed in a window above the + button. There are a few limitations. The maximum amount of material you can add is displayed directly above the + button.
Another way to increase the odds of an upgrade succeeding is to use an item known as the Blacksmith Blessing or an upgraded version of it. This item is beneficial in the event that you have a weapon or piece of equipment that has reached the highest level for its tier or is close to it. This item allows you to upgrade the equipment to the next level without risking its breaking.
If an upgrade fails, it will be reverted to its previous state. This is typically a downgrade, but could also be an upgrade. You should save an upgrade until you reach the maximum level of an Best item upgrader.
